We are looking forward to serving you, but to make that happen faster, please fill out this form below with as much information that you can.  While you can call us, we recommend using this process to speed up your booking and design request.  It saves time with paperwork when you visit as well. Do not skip any section and do not continue until you have read and answered all questions and verified that you have correctly submitted. Your appointment will NOT proceed without filling out the form correctly.

You must be 18 or older to book any type of appointment! Minors can be tattooed or pierced with parental/guardian consent and a birth certificate, but AN ADULT MUST COMPLETE THE BOOKING. Minors are not pierced or tattooed in intimate or suggestive areas, no exceptions. Review Pricing policy or our Minor Policy for more information.

We allow but discourage guests. Guests are generally unnecessary and distracting to the artist and client. They may not be permitted in the working area, depending on the discretion of the artist. We are limiting exposures and contacts that are not necessary. Exceptions will be made for accompanying nurses/healthcare workers for handicapped customers and parents of minors.

***Minors are expected to be able to have work done alone. If the child is incapable or uncomfortable being alone with artist, they will not be able to get work performed and deposit will be forfeit; this applies to piercing and tattoos. In rare cases we will approve of a parent being present, but this is at the discretion of the owner, artist, and space that is available. Parents and all guests are expected to wait in the lobby unless space permits by default.
